The core difference: coordinated care vs. open access
TRICARE Prime and TRICARE Select are both available to eligible beneficiaries in the Fayetteville / Fort Bragg area, but they work in fundamentally different ways. The choice shapes how you find doctors, whether you need referrals, and what you pay.
TRICARE Prime is a managed care plan. You are assigned a primary care manager (PCM) who coordinates your care, handles most routine visits, and writes referrals when you need a specialist. In exchange for that coordination, Prime generally has lower out-of-pocket costs.

There is an important additional step in the Fort Bragg area: the Right of First Refusal. When a referral is submitted, Womack Army Medical Center gets the first opportunity to provide that specialty care. If Womack can accommodate you, your referral will be directed there. One notable exception under Prime: outpatient mental health care from a network provider does not require a referral. You can schedule those visits without going through your PCM first.
Provider access under TRICARE Select
Under TRICARE Select, you can see any TRICARE-authorized provider without a referral for most primary and specialty care. You do not need a PCM, and you are not subject to the Right of First Refusal process.
Some services still require pre-authorization from Humana Military before you receive them, even under Select. Pre-authorization is separate from a referral: it is an advance approval for a specific procedure or treatment. Because Select allows out-of-network visits (at higher cost), confirming whether your provider is in-network is still important. In-network providers have contracted rates that reduce your cost-share; out-of-network visits cost significantly more.

TRICARE Prime typically has lower out-of-pocket costs than Select for in-network care. Active-duty service members enrolled in Prime generally have no cost-sharing. Family members and retirees enrolled in Prime pay enrollment fees and modest copayments for most visits.
