The ground rules, in plain English.

How to use ProviderQuoHealth responsibly — a directory helping patients find qualified healthcare providers, and providers connect with the patients searching for them.

Effective date: April 28, 2026. These Terms of Service are an agreement between you and ProviderQuoHealth LLC, a North Carolina limited liability company (“ProviderQuoHealth,” “we,” or “us”), and govern your use of providerquohealth.com (the “Site”) and the ProviderQuoHealth directory service. By using the Site you agree to these terms. If you do not agree, please do not use the Site.

Not Medical Advice

ProviderQuoHealth is a directory, not a healthcare provider. Nothing on the Site constitutes medical advice, diagnosis, or treatment, and the Site does not create a doctor-patient relationship between you and any provider listed here. Always consult a qualified healthcare professional for medical questions, and call 911 (or your local emergency number) in an emergency.

1. Acceptance of Terms

Using this Site — whether you're browsing the directory, submitting a listing, leaving a review, or anything else — means you accept these Terms of Service in full. This applies to all visitors, providers, and anyone who interacts with any part of the Site.

If you're submitting a listing on behalf of a practice, you also confirm that you have the authority to represent that practice and agree to these terms on its behalf.

2. Use of the Directory

You're welcome to browse and search the ProviderQuoHealth directory freely. Our goal is to make it as easy as possible for patients to find qualified healthcare providers. That said, the following uses are not allowed:

  • Scraping or automated access — do not use bots, scrapers, crawlers, or automated scripts to extract listing data in bulk without our written permission.
  • Data harvesting — collecting contact information from provider listings for unsolicited marketing, spam, or resale is strictly prohibited.
  • Misuse— using the Site in any way that could damage, overload, or impair our servers, or interfere with other users' experience.
  • Commercial redistribution — republishing or reselling directory content without our express written consent.

3. Listing Submissions

ProviderQuoHealth is a directory for legitimate, licensed healthcare providers and practices. When you submit a listing, you agree to the following:

Eligibility

Listings must represent a real, operating practice or individual provider who is licensed to practice in their listed jurisdiction and specialty. A valid NPI (National Provider Identifier) or current state license is required for verified status.

Accuracy

All information you submit — provider name, practice address, contact details, specialty, conditions treated, accepted insurance, and description — must be accurate to the best of your knowledge. You are responsible for keeping your listing up to date. Outdated or inaccurate listings that could mislead patients may be edited or removed.

Admin Rights

We review every listing before it goes live. We reserve the right to reject, edit, or remove any listing at our discretion — including listings that are incomplete, inaccurate, ineligible, or inconsistent with the mission of this directory.

False Claims of Licensure or Credentials

Falsely claiming licensure, board certification, or other credentials is a serious violation of these terms and may also violate state and federal law. Listings found to contain false credential claims will be removed immediately and may be reported to the relevant state licensing board.

4. Provider Verification

We use the NPPES National Provider Identifier (NPI) registry and state medical license records to verify that listed providers are real, licensed practitioners. Verified listings display a Verified Provider badge.

When you claim a listing, you may upload an NPI confirmation, state license certificate, or other credentialing document so we can confirm you are authorized to represent that provider or practice. Uploaded documents are processed in memory and discarded — they are not stored on our servers.

Misrepresenting credentials, submitting forged documents, or claiming a listing you do not have authority to represent is a violation of these terms. We reserve the right to report suspected fraud to the appropriate state licensing board or law enforcement.

5. Patient Reviews

Patients may post reviews of their experience with a listed provider. Honest reviews — including critical ones — are welcome. Reviews must be based on your own first-hand experience as a patient or the guardian of a patient. See our Review Policy for the full rules.

Do not include protected health information (PHI) in your review. Reviews are public and indexed by search engines. Anything you choose to disclose about your own care is your responsibility. We do not act as a HIPAA-covered entity, and reviews should be limited to your impressions of the experience — not detailed clinical history.

6. User Conduct

We built this site to help patients find good care and to give honest providers a place to be found. To keep it that way, all users must agree not to:

  • Submit spam, fake listings, or multiple listings for the same provider or practice under different names.
  • Harass, threaten, or intimidate any provider, patient, or other user.
  • Use the Site for any illegal purpose or in violation of any local, state, or federal law (including HIPAA, where applicable).
  • Impersonate another provider, individual, or organization.
  • Post content that is defamatory, discriminatory, or harmful to any user or the public.

Violations may result in removal of your listing or review and permanent ban from the Site.

7. Intellectual Property

Site Content

The ProviderQuoHealth name, logo, site design, editorial content, and all other materials created by us are © ProviderQuoHealth LLC. You may not reproduce, republish, or redistribute them without written permission.

Provider Listings

Your name, logo, photos, and description remain your property. We make no claim of ownership over the content you submit for your listing.

License to Display

By submitting a listing, you grant ProviderQuoHealth LLC a non-exclusive, royalty-free license to display, format, and distribute your listing information on the Site and in related promotional materials (such as social media posts or partner features) for the purpose of helping patients find your practice.

8. Disclaimers

ProviderQuoHealth is provided “as is.” We do our best to review listings before they go live, but we cannot guarantee that every listing on the Site is 100% accurate, current, or complete. Provider information can change — practices move, licenses lapse, specialties shift — and we rely on providers and public registries to keep listings updated.

We are not a healthcare provider, insurer, or referral service. We do not endorse any specific provider, and a listing on this Site is not a recommendation. You are solely responsible for choosing a provider, scheduling care, and evaluating whether a provider is right for your needs. Always verify licensure independently before booking an appointment — for example, through the relevant state medical board.

We make no warranty, express or implied, regarding the accuracy, reliability, or fitness for any particular purpose of the information found on this Site, including information about specialties, conditions treated, accepted insurance, or pricing.

9. Limitation of Liability

To the fullest extent permitted by law, ProviderQuoHealth LLC and its operators will not be liable for any indirect, incidental, or consequential damages arising from your use of — or inability to use — this Site or any listings on it. This includes any outcome of care, treatment, or interaction with a provider you found through this Site.

In plain English: we're a directory, not a clinic. Care decisions and their outcomes are between you and your provider. Our total liability to you for any claim related to this Site is limited to one hundred dollars ($100) or the amount you have paid us in the past twelve months, whichever is greater.

10. Third-Party Links

The Site may include links to external websites — such as a provider's own website, an insurer's site, a state licensing board, or the NPPES NPI registry. These links are provided for your convenience. We do not control external sites and are not responsible for their content, privacy practices, or accuracy. Linking to an external site does not imply our endorsement of it.

11. Governing Law

These Terms of Service are governed by the laws of the State of North Carolina, without regard to its conflict-of-law provisions. Any disputes arising under these terms will be resolved in the state or federal courts located in North Carolina.

12. Changes to Terms

We may update these terms from time to time as the Site grows and evolves. When we do, we'll update the effective date at the top of this page. For significant changes, we'll do our best to post a notice on the Site.

Continuing to use the Site after changes are posted means you accept the updated terms. If you don't agree with a change, you can stop using the Site and request removal of your listing by contacting us.
