What does a speech-language pathologist do?

Speech-language pathologists (SLPs) evaluate and treat communication and swallowing disorders. The range is wide: children with articulation, language, fluency (stuttering), or social-communication concerns; adults recovering from a stroke or brain injury that affects speech or language (aphasia); people with voice disorders, neurodegenerative conditions like Parkinson's or ALS, or swallowing difficulties (dysphagia); and people who use augmentative or alternative communication devices.

Most SLPs hold a master's degree (MA, MS, or MEd) and complete the ASHA Certificate of Clinical Competence โ€” a national credential involving 400 supervised clinical hours, a clinical fellowship year, and a national exam. Many sub-specialize in pediatrics, voice, swallowing, fluency, AAC, or specific clinical populations.

When to see an SLP

Common reasons: a parent's concern about a child's speech or language development relative to milestones; a pediatrician referral for articulation or fluency; an adult who has had a stroke and is having trouble finding words or following conversations; a swallowing evaluation after a hospitalization; voice changes that don't resolve; or progressive concerns with speech or swallowing in someone with a known neurological condition.

Early intervention matters: for children, the period before age three is especially responsive. The [Centers for Disease Control's developmental-milestone tracker](https://www.cdc.gov/ncbddd/actearly/milestones/index.html) is a reasonable starting point if you're not sure whether your child's speech is on track. Concerns are worth bringing to a pediatrician, who can refer to an SLP for a formal evaluation.

Most commercial plans, Medicare, and many Medicaid plans cover medically necessary speech therapy, often with a copay and an annual visit cap shared with physical and occupational therapy. Pediatric speech therapy is widely covered when there's a documented delay or disorder. School-based speech therapy for children with an Individualized Education Program (IEP) is provided through the school at no cost to the family.

Do I need a referral for an SLP?

Insurance plans vary. Medicare and many commercial plans require a physician referral; some allow direct access. Many pediatric SLPs work closely with pediatricians and developmental specialists, so the referral path is well-established. Confirm with your insurer before booking.

What's the difference between school-based and clinical speech therapy?

School-based speech therapy is provided through public schools at no cost to families for children with a documented educational need. It focuses on skills the child needs to access the school curriculum, and sessions are often in small groups. Clinical (medical) speech therapy, billed through health insurance, is typically individual, can address goals beyond the school context (swallowing, more intensive articulation work, AAC programming), and may be more frequent. Many children benefit from both running in parallel.
