What does a physical therapist do?

Physical therapists (PTs) evaluate and treat movement problems caused by injury, surgery, chronic conditions, or aging. Visits typically combine a physical exam, hands-on techniques (mobilization, soft-tissue work), supervised exercise, and a take-home program. PTs are licensed independent providers with doctoral-level training (DPT) and often work alongside physicians, surgeons, and chiropractors.

Common reasons to see a PT: recovery after orthopedic surgery, sports injuries, chronic back or neck pain, vertigo and balance issues, post-stroke rehab, pelvic floor concerns, and fall prevention. Many practices sub-specialize (sports, orthopedic, neurologic, pelvic health, pediatric), and matching the specialty to your concern improves outcomes.

Direct access vs. referral

Every U.S. state now allows some form of direct access to physical therapy, meaning you can usually start with a PT without a physician referral. The details vary โ€” some states have time limits or require a referral for ongoing care โ€” and your insurance plan may have its own referral rules even if your state doesn't. How long is a course of physical therapy?

Most plans of care run six to twelve visits over four to eight weeks, though some conditions need more and some need less. A PT should set goals at your first visit and re-evaluate progress every few weeks. If you're not improving, the plan should change or you should be referred elsewhere.

Is physical therapy covered by insurance?

Most commercial plans, Medicare, and many Medicaid plans cover medically necessary PT, typically with a copay and an annual visit cap. Maintenance visits and prevention-only programs are less often covered. Confirm the visit cap and any required referrals with your insurer before starting.

What should I bring to my first visit?

Bring any relevant imaging or test results, a list of your medications, and clothing you can move in (or comfortable workout clothes you can change into). The first visit is mostly evaluation: exam, history, goal-setting, and a starter exercise program. Hands-on treatment usually begins at the same visit.
